* 교수님의 서명 또는 도장이 날인 되어야 할 경우 반드시 담당 교수님께 직접 서명/도장을 받아 제출해 주십시오. 

  서명이 없을 경우 서류 접수가 불가능할 수 있음을 알려드립니다.
